Phyllis Carol Depp Cook died on Sunday, November 23 in the presence of her beloved husband of 51 years, John W. Cook. She is survived by her daughter, Stephanie Cook Ayles, her son, Clayton Cook and three grandchildren, Rylie, Cameron and Parker.
Phyllis had an amazing 45 year career as an occupational therapist. She began at Mass General Hospital in Boston, MA , then to Saint Raphael Hospital in New Haven. Her last ten years were in private practice in New Haven. Among her many honors, was most notably the Occupational Therapist of the Year Award presented to her in the late 1980s in Minneapolis. She was a graduate of Baylor University with a degree in Music and then received her Masters in Occupational Therapy from Texas Womens University.
Memorial Services will be held Friday, Nov. 28, at 3pm in First Congregational Church, 122 Broad St, Guilford. Burial will be held privately. In lieu of flowers, memorial contributions in Phyllis name may be made to a charity of choice.
